Teacher Recommendation Form

Wallin Scholarship Applicants must obtain at least two recommendations from current or past classroom teachers. An optional third recommendation may be from a community member who knows the student well, such as Admission Possible/Upward Bound staff, a minister, employer or coach.

If you are a teacher or community member who has been asked by a student to provide a recommendation, you may click here to download the recommendation form. After filling in the form, please save it to your computer and include the student’s first and last name in the title and email the completed form as an attachment to: wallin.staff@wallinfoundation.org.

OR, after filling out the form, you may print it out and give it to the designated person at the high school.
